Below is a link to a copy of COPE's "Statement of Understanding & Consent to EAP Services" Form.  This is the form we ask our clients to read during their first visit to our offices.

We will be adding additional information to this page. Once completed, it will collect a variety of assessment tests, forms, and other documents used in COPE's Employee Assistance services. Clients will then be able to review these documents online, or print them out for reference or use later.
